Our work in Chronic Disease Prevention catalyzes integrated, multicondition prevention systems that address the leading drivers of preventable illness and health-care costs, such as cardiometabolic risk. Through applied research and expert convenings, we connect cross-sector leaders to advance the adoption, scaling, and measurement of community-integrated prevention approaches. 

The Project Prevent Collaborative, anchored in Chronic Disease Prevention, brings together ecosystem leaders in a premier forum to move proven models into policy and everyday practice so prevention reaches more people.
