Primary care is the cornerstone of a resilient health system, providing preventive services, managing chronic conditions, and ensuring continuity of care. Yet in the United States, persistent underinvestment and workforce shortages have created major gaps in access, particularly in underserved urban and rural communities. To fill these gaps, a dynamic and often undervalued workforce, including nurse practitioners, physician associates, community health workers, and pharmacists, is delivering high-impact care. These providers are improving outcomes, reducing disparities, and meeting patient needs in ways traditional models cannot, yet they often are not being reimbursed accordingly. This session will explore how reimagining the care ecosystem through team-based models can drive improved community health. Experts will share successful strategies, identify policy reforms, innovative financing models, and highlight the potential of including underutilized partners in a more inclusive care infrastructure.
