Project Prevent Collaborative: Evidence-Based Models for Efficient Preventive Health Delivery (Invite only)

This in-depth session will showcase how evidence-based preventive health-care services can be scaled through deliberate partnerships using data and diverse community organizations and community-oriented providers. We will explore a community partnership approach that aligns the interests of neighborhood organizations, public health agencies, and health systems around shared goals through fit-for-purpose engagement. Speakers will demonstrate how data-informed outreach can identify populations most likely to benefit from timely screenings, disease management, and lifestyle interventions, allowing limited resources to be deployed more efficiently. Finally, we will highlight the pivotal role of non-physician providers, such as nurses, community health workers, and patient navigators, whose trusted relationships and firsthand understanding of social determinants make them indispensable for delivering preventive care where people live, work, and worship.
